To receive a Condo Questionnaire please fill out the form below. Standard Condo Questionnaires cost $150.00 and have a 5 day turn around. Next day Condo Questionnaires cost $200.00 and will be returned within 24 hours if the request is submitted before 11AM EST. Same day Condo Questionnaires cost $250.00 and must be submitted before 11AM EST. Condo Questionnaires cannot be processed until payment is received. To avoid delays in receiving your Condo Questionnaire, please make sure all required documentation is attached including a previously recorded deed showing the current owner’s name. If the owner is not the one requesting the questionnaire, than an Authorization Form is required. **If the closing does not take place on or before the closing date stated on the accompanying closing letter, then a new condo questionnaire and losing letter must be requested to insure additional charges have not accrued.**
